Entry Deadline for CAF 2007 Member Participants
Within ten business days of signing on as a CAF 2007 Member Participant, please send all forms (signed and completed Agreement and Waiver, and provide a current copy of your Certificate of Insurance) by mail or fax to:

For items (1) and (2):
Please print off two copies of each PDF document.
Sign and date all copies.
For item (3):
Please provide a current copy of your Certificate of Insurance.
Addendum to "CAF 2007 Certificate of Insurance Submission Guidelines":
ONCE THIS DOCUMENT IS SENT TO US, IF YOUR COMMERCIAL INSURANCE RENEWS ON OR BEFORE OCTOBER 31, 2007, THEN YOU MUST IMMEDIATELY SEND US A NEW CERTIFICATE OF INSURANCE TO PROVE THAT YOUR COMMERCIAL INSURANCE IS CURRENT.
Keep one set [of items (1), (2) and (3)] for your records and mail or fax one set to the Capital Arts Festival.
